Mayor Health MD
Terms of Use
Last updated: September 30, 2026
These Terms of Use govern your use of mayorhealthmd.com (the "Site"), operated by Mayor Health MD, PLLC ("Mayor Health MD," "we," "us"). By using the Site you agree to these Terms. Clinical care, memberships, fees, cancellations and refunds are also governed by our Policies, the Membership Agreement, and the consents you sign before treatment.
Not for emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. The Site, our scheduling tools and our messaging channels are not monitored continuously and must not be used for urgent or emergency needs.
Who provides care
All medical services are provided personally by Mark Mayoral, MD, licensed in Florida (License ME176710) and Mississippi (License 37596), NPI 1144925777. Telehealth services are available only to adults 18 and older who are physically located in Florida or Mississippi at the time of each visit.
No physician–patient relationship from Site use alone
Information on the Site is general and educational. It is not medical advice and does not create a physician–patient relationship. That relationship begins only after you complete intake, sign the required consents, and are evaluated by Dr. Mayoral. Booking a visit or purchasing a membership does not guarantee that any test, treatment or prescription will be provided; all clinical decisions are made in Dr. Mayoral's independent professional judgment.
Prescriptions
Medications are prescribed only when clinically appropriate after an individual evaluation, and are dispensed by a licensed pharmacy of your choice. Compounded medications are not FDA-approved; their risks and limitations are explained before therapy begins.
Accounts and payments
- You agree to provide accurate information and to keep your account credentials confidential.
- You must be the patient, or the patient's legally authorized representative, to book care.
- Fees and payment terms are shown at the time of purchase and in our Policies. Memberships renew automatically until cancelled as described in the Membership Agreement.
Acceptable use
You agree not to misuse the Site, including by submitting false information, attempting to access another person's account or data, interfering with the Site's operation, or using the Site for any unlawful purpose.
Intellectual property
The Site's content, design and branding belong to Mayor Health MD or its licensors and may not be copied or reused without permission.
Third-party services
The Site links to third-party services we use, such as our scheduling, payment and patient-portal providers. Those services are governed by their own terms and privacy policies.
Disclaimers and limitation of liability
The Site is provided "as is." To the fullest extent permitted by law, Mayor Health MD is not liable for indirect, incidental or consequential damages arising from use of the Site. Nothing in these Terms limits any liability that cannot be limited under applicable law, including professional obligations owed to patients.
Privacy
Our handling of information is described in our Website Privacy Policy and, for medical information, our Notice of Privacy Practices.
Governing law
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law rules, except where the law of the state in which you received care applies.
Changes
We may update these Terms from time to time. The current version will always be posted on this page with its "Last updated" date.
