Website Privacy Policy
Last updated: September 30, 2026
This Website Privacy Policy explains how Mayor Health MD, PLLC ("Mayor Health MD," "we," "us") collects, uses and protects information when you visit mayorhealthmd.com, book an appointment, or purchase a membership through this website. Medical information you share as a patient is protected health information (PHI) and is governed by our HIPAA Notice of Privacy Practices. If this policy and the Notice of Privacy Practices conflict regarding PHI, the Notice of Privacy Practices controls.
Information we collect
- Information you give us: name, email address, phone number, date of birth, mailing address and similar details you enter when you book an appointment, create a member account, purchase a membership, or contact us.
- Payment information: payments are processed by our payment processors (Square and, for some transactions, PayPal). We do not store full card numbers on this website.
- Technical information: our website host (Squarespace) automatically collects standard log data such as IP address, browser type, pages visited and referring site, and uses essential cookies to operate the site and member login.
Clinical intake forms, medical history and visit records are completed in our HIPAA-compliant electronic health record and patient portal, not on the public pages of this website.
How we use information
- To schedule and confirm appointments, manage memberships, and process payments.
- To respond to your questions and send administrative messages about your account or care.
- To operate, secure and improve this website.
- To comply with legal, regulatory and licensing obligations.
How we share information
We do not sell or rent your personal information, and we do not use health information for advertising. We share information only with service providers that help us operate the practice — such as our website host, scheduling tool (Acuity Scheduling), payment processors, electronic health record, secure messaging and laboratory partners — under contracts that limit their use of the information and, where PHI is involved, under a Business Associate Agreement. We may also disclose information when required by law or to protect the safety of a patient or others.
Cookies and analytics
This website uses cookies that are necessary for it to function (for example, keeping you signed in to the member area). We do not place advertising or tracking pixels on pages where patients book care or sign in. You can set your browser to refuse cookies, but parts of the site may not work.
Security
This website is served over an encrypted (SSL/TLS) connection. Patient communication takes place through HIPAA-compliant platforms. No method of transmission over the internet is completely secure, so please do not send medical information through the website's general contact form or standard email.
Your choices and rights
You may request access to, correction of, or deletion of the personal information we hold about you through this website, subject to the medical-record retention requirements of Florida and Mississippi law. You can unsubscribe from non-essential emails at any time. Rights regarding your medical records are described in our Notice of Privacy Practices.
Children
This website and our services are intended for adults 18 and older. We do not knowingly collect information from children under 13.
Changes to this policy
We may update this policy from time to time. The current version will always be posted on this page with its "Last updated" date.
