Mayor Health MD

Good Faith Estimate

Effective: October 2, 2026

Your right to a Good Faith Estimate

Under the federal No Surprises Act (45 CFR §149.610), health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of expected charges. Mayor Health MD is a direct-pay practice and does not bill insurance, so this applies to every patient.

  • You have the right to a Good Faith Estimate of the total expected cost of any non-emergency service.
  • When you schedule a service at least three business days ahead, you will receive your written estimate within one business day of scheduling (within three business days if you schedule ten or more business days ahead). You can also ask for one before you schedule.
  • If you are billed at least $400 more than your estimate, you can dispute the bill.
  • Keep a copy or picture of your estimate.

Our standard fees

All services are provided by Mark Mayoral, MD (NPI 1144925777), licensed in Florida (ME176710) and Mississippi (37596), by secure telehealth video, to adults physically located in Florida or Mississippi.

What an estimate does not include

  • Medications, including compounded medications, which are dispensed and billed by the pharmacy you choose. (Exception: Platinum core-protocol medications are included in the Platinum membership fee; they are prescribed under a patient-specific prescription and dispensed directly to the member by Empower Pharmacy, a state-licensed 503A pharmacy.) Compounded medications are not FDA-approved.
  • Laboratory tests, imaging, specialist care, or any service billed by an independent third party.
  • Unexpected costs if your care plan changes.

A Good Faith Estimate is not a contract and does not require you to obtain any service from us.

Disputing a bill

If you are billed at least $400 more than your Good Faith Estimate, you may start the federal patient-provider dispute resolution process within 120 calendar days of the date on the bill. There is a fee to use the process. If the reviewing agency agrees with you, you pay the estimated amount; if it agrees with the provider, you pay the higher amount.
