Welcome to Mayor Health MD. We are committed to providing transparent, patient-centered telehealth care to adults in the State of Florida. The policies below apply to all consultations, memberships, and services we provide. By scheduling, enrolling in a membership, or receiving care from Mayor Health MD, you agree to these policies.

1. Who We Are

Mayor Health MD is a Florida-based, telehealth-only direct-pay medical practice. Care is provided by Mark Mayoral, MD, a Florida-licensed physician (NPI 1144925777). Mayor Health MD does not bill insurance, Medicare, or Medicaid. All services are paid directly by the patient.

2. Telehealth Care

All services are delivered remotely through HIPAA-compliant video, secure messaging, and electronic fax. We do not use FaceTime, standard SMS text messaging, WhatsApp, Facebook Messenger, or personal email for clinical care.

Before your first visit, you will be asked to sign a Telehealth Informed Consent. You must also meet the following requirements:

  • Be physically located in the State of Florida at the time of each telehealth visit.
  • Be 18 years of age or older. Minors may participate only with the consent and participation of a parent or legal guardian.
  • Complete an intake form and sign all required consents before being seen.

Mayor Health MD does not provide hands-on examinations or in-office services. If your condition cannot be safely evaluated or treated by telehealth, you will be referred to an in-person provider, urgent care center, or emergency department.

3. Consultations

  • All visits are conducted by a Florida-licensed provider.
  • Consultation fees cover the provider's professional time and clinical review.
  • A consultation does not guarantee that any prescription, treatment, or referral will be issued. All clinical decisions are at the provider's independent professional judgment.
  • Completed consultations are non-refundable, even if no prescription is issued or the patient chooses not to proceed.

4. Prescription Medications

  • Prescriptions, when issued, are transmitted electronically to a pharmacy of the patient's choice.
  • We work with the patient's preferred local retail pharmacy and, where appropriate, with accredited 503A and 503B compounding pharmacies (including Empower Pharmacy).
  • Prescription decisions are individualized. We do not issue any medication without an individualized clinical determination of medical need.
  • Once a prescription has been transmitted to a pharmacy, changes or cancellations are not handled by Mayor Health MD; contact the pharmacy directly.
  • Medications cannot be returned or refunded once dispensed, in accordance with federal and state regulations.

5. Weight Management & GLP-1 Therapy

  • Medication-assisted weight management, including GLP-1 receptor agonists (such as semaglutide and tirzepatide), may be prescribed only after a clinical evaluation, review of medical history, and determination of medical appropriateness.
  • We may prescribe FDA-approved brand-name products or compounded versions dispensed by 503A/503B compounding pharmacies, where clinically and legally appropriate.

Important: Compounded medications are not FDA-approved. The risks, benefits, limitations, and applicable disclosures are explained in our Weight-Management & GLP-1 Therapy Informed Consent, which must be signed before therapy begins. Results vary; we make no guarantee of any specific amount of weight loss, timeline, or outcome.

6. Controlled Substances and ADHD

  • Mayor Health MD is not a pain-management clinic. We do not accept chronic non-malignant pain as a primary indication for new opioid prescriptions.
  • When clinically appropriate, controlled substances may be prescribed in compliance with the federal Ryan Haight Act and Florida law, including the required query of the Florida Prescription Drug Monitoring Program (E-FORCSE) under F.S. §893.055.
  • ADHD evaluation results in a treatment recommendation that may or may not include controlled-substance therapy, at the provider's clinical judgment. Completion of an evaluation does not guarantee a prescription.
  • Patients receiving any controlled substance must sign a Controlled Substance Patient Agreement and may be required to comply with monitoring, including PDMP review, urine drug screening, and pill counts.

7. Memberships

Silver ($20/month) and Gold ($100/month) memberships are governed by a separate Membership Agreement, which must be reviewed and signed before enrollment.

  • Silver — $20/month: reduced-fee weight-management consultations, secure patient portal messaging, prescription refill management as clinically appropriate, ongoing continuity of care.
  • Gold — $100/month: all Silver benefits, plus one included telehealth consultation per month (does not roll over), priority scheduling, expedited messaging response, and member-only discounts on programs and products.

Memberships are direct primary care agreements under F.S. §624.27 and are not health insurance. Membership fees do not cover laboratory testing, imaging, hospital care, emergency services, specialist care, surgery, the cost of any prescription medication or compounded product, or any service from a provider other than Mayor Health MD. We strongly recommend that all members maintain separate health insurance or other coverage for services not provided by the Practice.

8. Cancellations and Rescheduling

  • Appointments may be cancelled or rescheduled at no charge with at least 24 hours' notice.
  • Late cancellations and no-shows forfeit the appointment fee and are not eligible for refund or credit.
  • Late arrival of more than 10 minutes may result in a shortened or rescheduled visit at the provider's discretion. Time-based fees are not prorated for late arrivals.

9. Refunds

Detailed terms are in our Cancellation & Refund Policy. In summary:

Non-refundable:

  • Completed consultations.
  • Prescriptions once transmitted to a pharmacy.
  • Compounded medications and supplements once dispensed.
  • Membership benefits unused within their applicable billing cycle.
  • No-shows and late cancellations.

Refund-eligible:

  • Duplicate charges processed in error.
  • Cancellations made before any provider review has been performed.
  • Verified pharmacy shipment errors (subject to the pharmacy's policy and verification).
  • Prorated refund of unearned membership fees upon termination, as required by F.S. §624.27.

10. Good Faith Estimate (No Surprises Act)

If you do not have health insurance or are not using insurance, federal law (the No Surprises Act, 45 CFR §149.610) gives you the right to receive a Good Faith Estimate of the expected charges for your care. You will receive one in writing at scheduling or upon request.

If you are billed at least $400 more than your Good Faith Estimate, you may dispute the bill through the federal patient–provider dispute resolution process. Learn more at cms.gov/nosurprises or call 1-800-985-3059.

11. Privacy and HIPAA

Mayor Health MD safeguards your protected health information (PHI) in accordance with HIPAA and Florida law. Our full Notice of Privacy Practices is available on this site (see footer) and on request.

  • We use HIPAA-compliant electronic health record, secure messaging, and telehealth platforms, each under a Business Associate Agreement.
  • We do not use FaceTime, standard SMS, WhatsApp, Facebook Messenger, iMessage, or personal email for clinical communication.
  • If you contact us through a non-secure channel, we will respond through our secure patient portal.

Certain categories of information receive additional protection under Florida law, including HIV/AIDS status, mental health treatment records, substance use disorder records, and genetic information. We apply the more protective of federal or Florida law when handling those records.

12. Age Requirement

Patients must be at least 18 years old, or have written consent and participation from a parent or legal guardian.

13. Emergencies

Mayor Health MD does not provide emergency services. In a medical emergency, call 911 or go to the nearest emergency department. We do not provide 24/7 on-call coverage or after-hours urgent care.

14. Required Patient Forms

Before your first visit (or before your next visit if you are an existing patient), you will be asked to review and sign:

  • Notice of Privacy Practices (HIPAA)

  • Telehealth Informed Consent
  • Financial Policy & Fee Agreement
  • Cancellation & Refund Policy
  • Patient Rights & Responsibilities
  • Good Faith Estimate (acknowledgment of receipt)
  • Membership Agreement (if enrolling in Silver or Gold)
  • Weight-Management & GLP-1 Therapy Informed Consent (if pursuing weight-management therapy)
  • Controlled Substance Patient Agreement (if receiving any controlled substance prescription)

15. Changes to These Policies

We may update these policies from time to time. The most current version will always be posted on this page with an updated "Last updated" date. Material changes affecting existing memberships will be communicated to members in advance, in accordance with the Membership Agreement.
