Membership Agreement

Effective: October 2, 2026

This Membership Agreement is between Mayor Health MD, PLLC ("the Practice"), 3531 SW 87th Ct, Miami, FL 33165, and the member named on the signature page. It is a direct health care agreement under section 624.27, Florida Statutes. Members review and sign this agreement before enrollment.

1. Who provides care

All services are provided personally by Mark Mayoral, MD, licensed in Florida (ME176710) and Mississippi (37596), by secure telehealth. Members must be 18 or older and physically located in Florida or Mississippi at the time of each visit.

2. Plans and fees

Fees are charged in advance on the enrollment date and on the same date each month or year after that.

3. Services covered by the membership fee

Gold: one telehealth consultation per calendar month (unused visits do not roll over); priority scheduling; a secure messaging line to Dr. Mayoral for questions between visits, answered as availability permits; prescription refill management and medication adjustment as clinically appropriate; and 30% off eligible medications prescribed by the Practice and dispensed through Empower Pharmacy.

Platinum: everything in Gold, plus up to five telehealth consultations per calendar month (unused visits do not roll over), phone access to Dr. Mayoral, a core protocol of medications or supplements when clinically indicated after evaluation, 50% off eligible medications outside that protocol through Empower Pharmacy, and member merchandise. The core protocol is set by Dr. Mayoral based on clinical need, is prescribed only after evaluation, and is dispensed by a licensed pharmacy.

4. Services not covered, and their fees

The membership fee does not cover laboratory tests, imaging, hospital or emergency care, specialist care, surgery, or any service from another provider. Medications are not covered, except a Platinum member's core protocol. Platinum core-protocol medications are prescribed under a patient-specific prescription, dispensed directly to the member by Empower Pharmacy (a state-licensed 503A pharmacy), and paid for by the Practice as part of the Platinum fee; the member is not billed separately for them. Consultations beyond the included number are billed at 30% off the standard fee for Gold members and 50% off for Platinum members. Standard fees are listed on our Good Faith Estimate page.

5. Controlled substances

A membership never guarantees that any medication will be prescribed. Controlled substances are prescribed only when clinically appropriate and legally permitted, after PDMP review and a signed Controlled Substance Patient Agreement. Patients on controlled-substance therapy must keep an active membership, which covers the required follow-up visits, PDMP review and ongoing monitoring. The membership fee pays for that care and monitoring, never for a prescription.

6. Messaging is not emergency care

The messaging line is not on-call or urgent-care coverage. For a medical emergency, call 911.

7. Term and renewal

Monthly plans renew automatically each month and annual plans each year, at the then-current fee, until either party ends the agreement under section 8. We will give at least 30 days' written notice before any fee change.

8. Ending the agreement

Either party may end this agreement with at least 30 days' written notice to the other (email to mark@mayorhealthmd.com is enough). The Practice may end it immediately for a breach of this agreement or a violation of the physician-patient relationship. On termination, we refund any membership fees paid in advance for periods after the termination date, prorated by month. See our Cancellation & Refund Policy.

9. If the Practice stops providing care

If the Practice stops offering health care services for any reason, we will refund all membership fees paid in advance for any period after services stop.

10. Records and privacy

Your health information is handled under our Notice of Privacy Practices.

11. Governing law

This agreement is governed by Florida law.

Required disclosure

This agreement is not health insurance and the health care provider will not file any claims against the patient's health insurance policy or plan for reimbursement of any health care services covered by the agreement. This agreement does not qualify as minimum essential coverage to satisfy the individual shared responsibility provision of the Patient Protection and Affordable Care Act, 26 U.S.C. s. 5000A. This agreement is not workers' compensation insurance and does not replace an employer's obligations under chapter 440.

The signed copy of this agreement, including the member's plan, billing choice and signature, is completed at enrollment.
